Social Security Bar

Social Security officially began in 1937. The Social Security Death Index is a  computerized index to deaths reported and/or death benefits paid out starting in 1962. When a person applied for a Social Security number, he filled out a form call the SS-5. Although the SS-5 forms have changed throughout the years, the most common information included in the form are name, present mailing address, age at last birthday, date of birth, place of birth, father's full name, mother's full name, sex, race, address of employer, and signature. Rootsweb has online a Guide to Using the Social Security Death Index which describes how to use the index, how Social Security numbers are assigned, how to obtain copies of an original SS-5, etc.

If you have obtained an SS-5 form for a deceased individual who was born in or lived in Loudon County, please consider adding it to the database for other researchers to enjoy.
