Effective February 15, 2005, anyone seeking a certified copy of a birth or death certificate from a Vital Records office must have either a signed and notarized application or appropriate identification. The preferred type of identification is a government issued identification card signed by the applicant. Without government issued identification, other forms of ID and/or affidavits may be acceptable.

• Births & Deaths Before 1908

Tennessee did not require formal records before 1908. Four cities did keep some earlier birth and death records, that are available through the Tennessee State Library & Archives (Memphis from 1848, Chattanooga from 1872, Nashville from 1874, Knoxville from 1883).

Tennessee residents may write for a free search (fee $2 if found). Non-residents are charged a $5 search fee. A three-year period can be searched for this fee.

• Births & Deaths 1908 - 1912

These records were kept by the Scott County clerks. They have been microfilmed and are available at the Tennessee State Library & Archives. Tennessee residents may write for a free search (copy fee $2 if found). Non-residents are charged a $5 search fee.

• Births & Deaths 1913

The law requiring birth & death registration expired in 1913, and no records were kept for that year.

• Births 1914 - Present (Long Form Copies)

These records are available from the Office of Vital Records. The fee for a search of the records is $10.

• Births 1949 - Present (Short Form Copies)

These records are available from the Office of Vital Records. The fee for a search of the records is $5.

• Deaths 1914 - 1945

These records are available from the Tennessee State Library & Archives. Tennessee residents may write for a free search (fee $2 if found). Non-residents are charged a $5 search fee. A three-year period can be searched for this fee.

• Deaths 1946 - Present

These records are available from the Office of Vital Records. The fee for a search of the records is $5. Always provide NAME, YEAR and COUNTY when requesting a search.
