I, William E. Blakeley do solemnly swear that I served as a soldier in the army or [navy] of the Confederate States, being a member of 4 th Tennessee Cavalry [Capt. Sam Glover] Regiment 4 th Tennessee Cavalry from the State of Tennessee or member of the crew of the ship called………………………; that I was honorably discharged [paroled or released] from such service on or about the 3 rdday of May 1865 and did not desert the same; that I was am now for the past twelve months have been, a bona fide resident of this State; that I do not myself, nor does my wife, nor do we both together, own property, real or personal or both, or money or choses action in excess of the value of $100.00 [exclusive of household goods and wearing apparel] nor has either of us conveyed title to any property to enable me to draw a pension and that neither I nor my wife is in receipt of any income, annuity, pension, or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of ordinary avocations of life

Confederate Soldiers Home of Arkansas
To the Board of Directors;
I have arrived at an age and condition in life that I am no longer able to support myself and I hereby apply for admission in the Soldiers Home. My full name is William E. Blakely. I am 78 ½ years old. I have 0 living children; sons,…..daughters…… I have been married 1 times; my wife is [living or dead] Dead . should any accident happen to me please notify
C. B. M. McArthur whose address is Morrilton, Arkansas. My residence is Bentley Township, County of Conway State of Arkansas. I was regularly enlisted and served in the war of 1861-1865 in the Confederate army [navy] as follows: I enlisted in the month of August 1861 at Chattanooga in the State of Tennessee in Captain C. C. Spillers Company’s afterwards Co. “H“ 4 th Tennessee Cavalry Regiment, Tennessee Volunteers in General Thomas Harrison Brigade, Army of Tennessee. I was in 3 battles. The name of these battles are Battles of Perryville, Kentucky, Battle of Murfreesboro, Battle of Chickamauga and other smaller battles.
If wounded, state in what engagements, at Never wounded
I served honorably during the war in said army [or navy] and was true and loyal to the government of the Confederate States to the end of the war. When the war closed I was a Charlotte, North Carolina, paroled at Charlotte, North Carolina, May 3, 1865.
I further swear that I am disabled and indigent; and am not able to support myself. I do not own money or property or both in my own name, or that of another, to amount of $400.00; and my income does exceed $150.00 a year. I was never a deserter at any time during the war. I have been a resident of the State of Arkansas for one year preceding this date. I have not drawn a pension since September 1915. I am a person of sobriety and good habits; and if admitted to the Home as an inmate, I hereby promise to abide by and obey all rules and regulations made for the government of the Home; and that I will perform all duties required of me and obey all lawful orders of the officers of said Home. I sign this application with full knowledge that any violations of any of the rules will subject me to suspension or dismissal at the discretion of the Board of Directors.

Physicians Certificate
TO THE MEDICAL EXAMINER
Dear Doctor: Your careful and conscientious attention is requested. The law provides that only such veterans are as disabled to the extent that they are unable to support for themselves can be admitted. On account of a generous wish to help the unfortunate many physicians certify to disability in general terms, who would not do so on careful analysis. This entails much hardship as no inmate is retained in the Home if he found able to earn a support, and the Board of Mangers wish to extend help only to those who need it.
