 INSTRUCTIONS FOR COMPLETING FORMS

Photo-copy each of the forms before you start (in case of entry error). You will need several of the Family Group Sheets-- one for each husband & wife ("family") listed on Pedigree Chart.

IMPORTANT: list all dates by day, then month (letters- not by month number), then four digit year: example- "28 Jan 1895".

List all of the women by their BIRTH (not married) names.


PEDIGREE CHART

Please PRINT all information. This chart is designed to show the direct lineage of the family.

        (1) List yourself in #1 position by your birth name, date of birth, where born, date married, where married.
                        Below that is an area for you to list your present spouse, if any.

        (2) List your father's information at position #2

        (3) List your mother's information, by maiden name (if known) at position #3

        (4) List your father's parents in positions #4 & #5

Continue on,  completing each generational level by using the above as a guide.  Go as far as you can go.

If you run out of space on a particular family line, then start a second chart- using the last person that you entered on this chart as "#1" on the second chart.

Enter as much information as you can as to dates of birth, death & marriage, as well as location of each "event".


After you complete the above chart, you will be able to compute how many of the Family Group Sheets you will need to photo-copy, using one for each family. The Family Group Sheets are designed to show the family as a unit.


FAMILY GROUP SHEET ("FGS")

(FGS-1) The first one is for your immediate family. List the SAME person that is shown on Pedigree Chart as #2 (your father) as "HUSBAND" and the SAME person that is shown on chart as #3 (your mother) as "WIFE".
Beginning with their oldest child, list yourself and all of your FULL brothers & sisters in the lower section, continuing to the "2nd Page", if necessary.

(FGS-2) This form should list your father' parents (#4 & #5 on Pedigree Chart) as the "husband" & "wife", your father (#2) and all of his full brothers & sisters in the lower space as "children", beginning with the oldest known child.

(FGS-3) This form should list your mother's parents (#6 & #7 on Pedigree Chart) as the "husband" & "wife", your mother (#3) and all of her full brothers and sisters in the lower space as "children", beginning with the oldest known child.

(FGS-4) This form should list your father's father (#4 on Pedigree Chart) as a child, his parents (#8 & #9 on Pedigree Chart) as the "husband" & "wife", and all of his full brothers & sisters, beginning with the oldest known child.

(FGS- 5)... This form should list your father's mother (#5 on Pedigree Chart) as a child, her parents (#10 & #11 on Pedigree Chart) as the "husband" & "wife", and all of her full brothers & sisters, beginning with the oldest known child.

(FGS- 6)... This form should list your mother's father (#6 on Pedigree Chart) as a child, his parents (#12 & #13 on Pedigree Chart) as the "husband" & "wife", and all of his full brothers & sisters, beginning with the oldest known child.

(FGS-7)... This form should list your mother's mother (#7 on Pedigree Chart) as a child, her parents (#14 & #15 on Pedigree Chart) as the "husband" & "wife", and all of her brothers and sisters, beginning with the oldest known child.
